The Georgia POLST


Physician Orders for Life Sustaining Treatment (POLST) is a medical order that aims to enable seriously ill patients to designate the treatments they want and to ensure that those preferences are honored by medical professionals.

The Georgia POLST Collaborative is a part of the National POLST Paradigm and aims to educate patients, families and health care professionals throughout the state about the GA POLST.

2015 POLST Conference Update
Alliant ASO - www.AlliantASO.Org The 2015 Georgia POLST Conference took place in Atlanta in April. From the left are Richard Cohen, M.D. (a MAG member, the facilitator of the Georgia POLST Collaborative, and the chief medical officer for the WellStar Health System ethics program), Ira Byock, M.D. (the keynote speaker), and Adrienne Mims, M.D. (a MAG member and the chief medical officer for Medicare Quality Improvement with Alliant GMCF). Dr. Byock gave a dynamic talk on Providing "The Best Care Possible" Through the End of Life. A Georgia POLST form and additional information on the Georgia POLST Collaborative, which "aims to educate patients, families and health care professionals throughout the state about the Georgia POLST” is available here on our site.
